Job Summary:
Homeside Rehab is seeking compassionate and skilled Pediatric Physical Therapists (PTs) and Occupational Therapists (OTs) to join our home health team. In this role, you will provide individualized, evidence-based therapy services to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.
Job Responsibilities:
Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
Provide evidence-based PT and OT interventions to improve motor skills, mobility, functional independence, strength, balance, and developmental outcomes
Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
Collaborate with the clinical team to ensure coordinated and effective treatment
Communicate clearly with office staff, patients, and caregivers regarding progress and care plans
Active New York State Physical Therapy or Occupational Therapy License — required
Basic computer proficiency for electronic documentation
Strong clinical assessment and interpersonal communication skills
Ability to work independently in a home health environment
Valid New York State driver’s license and reliable transportation for home visits
Pediatric experience is preferred
Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings
Job Type: Full-time, Part-time, Per Diem
Location: Bronx, NY
Salary: $65 – $80 per hour
Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
